- Hospitalized adults with Prediabetes, type 1, type 2, or stress/steroid-induced hyperglycemia across medical, surgical, and critical-care units.
- Outpatients with newly diagnosed diabetes or those requiring medication, technology, or lifestyle adjustment in primary care and specialty clinics.
- Pregnant and postpartum individuals with gestational or pre‑existing diabetes in inpatient .
- Individuals with complex diabetes and multiple comorbidities (e.g., cardiovascular disease, chronic kidney disease, obesity) transitioning between inpatient, outpatient, and post‑acute care.
- Culturally and socioeconomically diverse patients, families, and caregivers who benefit from tailored diabetes self‑management education and support across the continuum of care.
Focus Areas
- Delivers individualized diabetes self‑management education for both inpatients and outpatients.
- Teaches blood glucose monitoring, insulin and non‑insulin therapies, and sick‑day management.
- Provides nutrition, carbohydrate counting, and physical activity counseling across care settings.
- Educates on preventing and recognizing acute and long‑term complications, including when to seek care.
- Coordinates discharge and follow‑up education to ensure safe transition from hospital to home or clinic.
Biography
Alice Scott is a Registered Nurse and Certified Diabetes Care and Education Specialist (CDCES) with extensive experience providing both inpatient and outpatient diabetes education. With a master’s degree in education. She combines strong clinical expertise with advanced teaching skills to support individuals and families in understanding and managing diabetes.
In the hospital setting, Alice collaborates with the interdisciplinary team to provide individualized diabetes education, medication teaching, and discharge planning to promote safe transitions of care.
In the outpatient environment at Grant Medical Centers DSME program, she leads comprehensive diabetes self-management education and support, focusing on blood glucose monitoring, nutrition, medication management, and lifestyle modification.
